If you have a family doctor or access to a walk-in clinic, please have your doctor or nurse practitioner (NP) fax a consult to Dr. Hoffart at 780-438-7710. If you need to see a walk-in doctor for a referral, please feel free to book with our Locum Doctors at Shifa Medical Clinic and request referral to Dr. Hoffart. If you don't have access to a family doctor or just have questions for us, feel free to self-refer by using our online contact form page (unless you are requesting a Nexplanon removal, as a referral is needed). All inquiries are strictly confidential. Please expect a call back within 1 week of submission - of note, the call may come from a PRIVATE number.
